Reasons for denial include, but are not limited to: having already received the maximum allowed amount of a covered item, duplicate authorization requests, insufficient documentation, or the determination that the requested item is not needed. If you have received an email stating that authorization is denied, please reach out to us by responding to the email you received or contacting our Customer Care team. We will review the denial and determine if we are able to resubmit the claim.
